SOCIAL WELLNESS SURVEY

AN AFFECTIVE INTERFACE FOR INVESTIGATING THE SOCIAL WELLNESS
OF YOUNG MALAYSIANS AND KOREANS

 
The questionnaire is anonymous. Do not write your name on it.
Please read each question carefully and tick whichever response is appropriate.

The questionnaire will be treated as strictly confidential.

General information:

This survey is designed to help us gain an insight into young adults’ social well-being. It also helps us to understand the relationship between social well-being and health status.

The survey consists of seven sections. Section A involves completing 9 demographic items. Please complete Section A before moving on to the other sections.

Section B consists of 3 questions asking the participants on their understanding of social well-being.

Section C is comprised of 15 questions on social well-being. Please select a response that best reflects your own reaction to it.

Section D consists of 7 questions on general health. Please select the best answer.

This questionnaire is not a test; therefore, there is no right or wrong answer. Please respond to the statements honestly.

Being in this study is voluntary and you are under no obligation to participate. However, if you do consent to participate, you may only withdraw prior to the questionnaire being submitted. Once your questionnaire is submitted you CANNOT withdraw from this study, as this is an anonymous study and so the identification and extraction of your questionnaire once submitted will not be possible.

Your confidentiality and anonymity are assured. Return of the completed survey to the website provided is implying consent because the survey is anonymous.
